Register of Birth
When a child was born, you should report the fact to municipal offices.
Foreigners who reside in Japan also report the birth in accordance with the provisions of Family Register Law if the child was born in Japan.
In addition to the report of birth, the new born child needs foreigner registration and acquisition of qualification for sojourning. Please do these reports along with the report of birth.
Please report the case of child birth to the home government, too. Please ask embassy or consulate about the procedures.

Required Documents
- Registration Form of Birth (municipal offices or hospitals have it)
- Birth Certificate (hospital or midwife’s approval upon delivery)
- The Seal of the Person who Submits the Report (for one who doesn’t have one, it is okay with his or her signature)
- Mom-Baby Health Diary(that mother receives as she submits Report of Pregnancy to the municipal office she resides when she knows that she is pregnant).
- National Health Insurance Card(only for subscribers)
Receipt Certificate of the Report of Birth
- ocument that certifies the receipt of the report of birth.
